H. Pylori Stool Antigen Test

H. pylori is a common cause of peptic ulcers and can lead to stomach cancer if untreated. The stool antigen test detects H. pylori bacteria in a stool sample and is used to identify an active infection in the stomach. Because it looks for bacterial antigens rather than pylori antibodies, stool tests reflect a current helicobacter pylori infection rather than past exposure. This makes it a reliable option both for initial diagnosis and for confirming whether H. pylori has been successfully treated after therapy is completed.

What to Expect

Preparation
You may need to stop antibiotics, proton pump inhibitors, and bismuth-containing products at least two weeks before the test.
During the Test
You’ll provide a stool sample.
After the Test
Results will indicate the presence of H. pylori infection.

Rapid Urease Test

The rapid urease test detects H. pylori during an upper gastrointestinal endoscopy by analyzing a small tissue sample taken from the stomach lining. The sample is placed in a testing medium that reacts to urease, an enzyme produced by H. pylori. When the enzyme is present, it triggers a measurable change that indicates infection. Because the test is performed directly on stomach tissue, it allows for prompt identification of H. pylori.

What to Expect

Preparation
You’ll need to fast for several hours before the endoscopy.
During the Test
A small sample of your stomach lining is taken and tested for urease activity, which H. pylori produces. The procedure takes about 15-30 minutes.
After the Test
You can resume normal activities once the sedative wears off.

Urea Breath Test

The H. pylori breath test measures carbon dioxide in your breath after you drink a urea solution. H. pylori bacteria break down urea in the stomach, producing carbon dioxide that can be detected in the breath. When this process occurs, it indicates the presence of an active infection rather than past exposure.

The breath test is non-invasive and does not require endoscopy.

What to Expect

Preparation
You may need to avoid certain medications and foods before the test.
During the Test
You’ll drink a urea solution and then breathe into a collection bag. The test takes about 15-30 minutes.
After the Test
Results will indicate if H. pylori is present.

Helicobacter Pylori Test Advantages and Limitations

Helicobacter pylori testing is useful because it identifies an active infection that can be treated directly, rather than inferring a cause based on symptoms alone. Stool antigen and breath tests are non-invasive and well suited for patients who do not otherwise require an endoscopy. When performed under the right conditions, these tests are reliable and help confirm whether H. pylori is contributing to symptoms or peptic ulcer disease. Testing during endoscopy offers the advantage of direct sampling when a closer look at the stomach lining is already needed.

At the same time, H. pylori testing has clear limitations. Results can be affected by recent use of antibiotics, acid-suppressing medications, or bismuth-containing products, which may lead to false-negative findings if preparation guidelines are not followed. A positive test confirms infection, but symptoms can have multiple contributing causes. In other words, not everyone with H. pylori experiences significant discomfort, and other conditions may also be present. Conversely, a negative result does not rule out other causes of stomach pain, nausea, or reflux-related symptoms.

Because of these limitations, results are always interpreted in clinical context.

Helicobacter Pylori Test Advantages and Limitations

Patients with ongoing stomach symptoms, a history of ulcers, or digestive discomfort that has been difficult to explain may benefit from H. pylori testing. Identifying whether this infection is present can help clarify the cause of symptoms and guide appropriate treatment. At our gastro clinic in Montreal, testing is approached as part of a broader clinical evaluation rather than as a standalone result.

In some cases, non-invasive testing such as a stool antigen or breath test is sufficient. In others, H. pylori testing may be performed during endoscopic procedures, particularly when upper gastrointestinal endoscopic procedures have been already planned to assess ulcers, inflammation, or other findings in the stomach. This approach allows testing to align with the overall diagnostic plan without adding unnecessary steps.
